How it runs

Chronic care management, run with Chief.

Chief works the enrollment queue with your team: from the first eligibility check to a month that is ready to bill. People stay in charge of every decision.

  1. 1

    Eligible

Chief checks diagnoses in the chart against each program's rules and lists the patients who may qualify.

  2. 2

    Consent call

Chief calls to explain the program and what it may cost, and asks for consent. Consent is required to enroll. Calls stay inside your calling hours, with up to two tries.

  3. 3

    First visit booked

Right after consent, Chief offers to book the first visit on the same call. It is optional. If the patient says no, your care manager gets a task and the patient gets a text with a booking link.

  4. 4

    Monthly care time

A care timer records your team's time with each patient through the month.

  5. 5

    Ready to bill

Billing shows progress toward each CPT code and flags what is still missing before the claim.

Helpers

Watch the helper dots take the load.

One person, a stack of tasks. The helpers lift them off, work on each one, and check it off the list. They are Chief's helpers for the calls, texts, follow-ups and bookings.

Helpers take the repetitive work: calls, texts, follow-ups and bookings.

Your team approves before anything goes out.

The care stays human.

The apps

Three apps. One connective intelligence.

Each person gets a space built for their part of care. Chief connects the dots between them.

Care managers and care teams

NurseDot

Coordinate ongoing care: patients, tasks, messages, calls, care plans and care management.

Physicians and providers

DoctorDot

Review a patient's context, Chief's summary and intervention reports, next to the EHR or on the web.

Patients

PatientDot

Goals, lessons, food and symptom logging, chat with the care team, and care plan, in one place. No separate app-license fee.

Chief AI

The co-pilot for your care team.

Chief is the connective intelligence behind CareDots, not a separate chatbot. It writes patient summaries, raises Watchlist findings for your team to approve or decline, makes enrollment, check-in and outreach calls, and prepares intervention reports.

  • Voice. Patients can talk to Chief in PatientDot.
  • Camera. A photo of a meal is read and logged.
  • Helpers. The helper dots take the busywork.

It supports people. It does not diagnose or treat on its own.

What Chief AI does

AI Scribe

Talk to the patient. The scribe does the rest.

One scribe for check-in calls, care planning, assessments and visits. It listens on the phone, on video or in the room, and turns the conversation into work your team can review.

  1. 1

    Call guides

A guide keeps each call on track. Topics tick off live as they come up, so nothing is missed.

  2. 2

    Notes

A draft note in your template, ready to review and edit. Earlier versions are kept.

  3. 3

    Goals and care plan

Goals, focus areas, problems and interventions agreed on the call are pulled out for the care plan.

  4. 4

    Tasks

Follow-ups mentioned on the call become tasks, plus a message for the patient and one for the care team.

  5. 5

    Assessments

Questionnaires fill in as you talk, with scores worked out and anything that needs a look flagged.

  6. 6

    Quality review

Each call gets a quality score, so leads can coach the team. Scores for a colleague's calls are for admins only.

Conditions, medication changes and readings heard on the call are suggested for the chart. A person adds or dismisses each one.
